• hyperlipidemia

• chronic kidney disease

 

the chronic care management program (ccm) could be for you. the goal is to keep you healthier between visits, to reduce your sick visits and trips to the er as well as reduce any hospital admissions.

 

our nurse, Maureen, will contact you between visits to make sure your health needs are met.  in addition, you will have 24-hour access to her by phone with a direct number that only those enrolled in CCM have access to.  

 

your insurance covers this service, minus a small co-pay that is under $10 per month.
